If you are a California resident submitting a request to opt-out of the sharing and sale of personal information pursuant to the draft revisions of California’s Privacy Rights Act (CPRA), you may exercise your rights by completing this Opt-Out Form. Non-California residents do not have such rights under the CCPA/CPRA.

While Market Proz will do all it can to honor your request, we may not be able to in certain circumstances. For example, if we cannot identify you within our systems, we will not be able to honor your request.

Participating sales agencies represent Medicare Advantage [HMO, PPO and PFFS] organizations and stand-alone PDP prescription drug plans that are contracted with Medicare. Enrollment depends on the plan's contract renewal.

We do not offer every plan available in your area. Currently, we represent 8 organizations which offer 68 products in your area. Please contact Medicare.gov, 1–800–MEDICARE, or your local State Health Insurance Program to get information on all of your options.
